1. Formulate life plans for seniors and rehabilitation plans for disabled individuals with physical or mental health conditions. Monitor and record the implementation of the customized plans
for each client. Modify treatment plans to comply with changes in clients' status. (30% of work time)
2. Provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Provide personal assistance, medical attention,
emotional support, or other personal care to clients and their families. Services include advising family care givers, providing clientsοΏ½ education and counseling, and making referrals for
other services. Also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare. (30% of work time)
3. Counsel clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life. Identify environmental impediments to client
or patient progress through interviews and review of patient records. Build up and organize clientsοΏ½ files and keep updated with actual progress. Establish and maintain interpersonal
relationship with clients, and develop constructive and cooperative working relationship with others. (20% of work time)
4. Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs. Utilize the data and evaluation report collected and organized to plan
and coordinate clientsοΏ½ care and rehabilitation, following through to ensure service efficiency and efficacy. Communicate with supervisor, peers or subordinates regarding the treatment,
status report and recommendations of the care giving services to clients. (15% of work time)
5. Conduct social research to advance knowledge in the social work field. Develop or advise on social policy and assist in community development. (5% of work time)
